CONSENT TO THE COLLECTION AND PROCESSING OF PERSONAL DATA
in accordance with Act No. 101/2000 Coll., on the protection of personal data, and Regulation (EU) 2016/679 of the European Parliament and of the Council
By submitting this form and clicking on the option "I agree," as the data subject, I give my consent to the processing of my personal data listed below, which I voluntarily provide to the data controller, in the manner specified in this consent. I acknowledge that these personal data concerning me constitute so-called personal data within the meaning of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC ("GDPR"). I also declare that I am at least 16 years old, and if not, I confirm that this consent has been expressed or approved by a person who exercises parental responsibility for me.
1. Controller
The controller of personal data is the company J. A. CLEAN spol. s r. o., with its registered office at Průmyslová 1888, 258 01 Vlašim, ID: 24736554, registered in the Commercial Register maintained by the Municipal Court in Prague, Section C, Insert 170064 ("Controller").
2. Processing of Personal Data
2.1. My personal data, which I voluntarily provide to the Controller, are as follows: Name, Surname, Email, Telephone number.
2.2. I give my consent to the processing of my above-mentioned personal data for the following purposes:

Marketing purposes of the Controller related to the offer of products and services; sending information about events, products, services, and other activities (e.g., in the form of newsletters, telemarketing); contacting for market research purposes and for marketing surveys; contacting for the purpose of sending Christmas and Easter or other holiday greetings and sending discount vouchers, gifts, etc.;
where the processing of personal data is necessary to fulfill these purposes.
2.3. The legal basis for processing is Article 6(1)(a) of the GDPR.
2.4. The Controller processes personal data that are adequate, relevant, and limited to what is necessary in relation to the purposes for which they are processed.
2.5. The above personal data will also be collected and processed by the following entities in addition to the Controller:

Recipients of personal data: The Controller is entitled to transfer the personal data of the data subject in particular to the following recipients and categories of recipients: suppliers of the Controller, employees of the Controller, providers of transport services, providers of marketing and advertising services, financial institutions, state authorities within the scope of fulfilling legal obligations established by relevant legal regulations, and other persons in a contractual relationship with the Controller.
The personal data of data subjects will not be transferred to third countries;
2.6. The Controller is authorized to process the above personal data for a period of 5 years from the granting of my consent. I acknowledge that before this period expires, the Controller may contact me to renew my consent, and that without its renewal, my personal data will be permanently deleted after the above period expires.
3. Rights of the Data Subject
3.1. I acknowledge that as a data subject, I have the right to request access to my personal data from the Controller (Article 15 GDPR), their rectification or erasure (Article 16 or Article 17 GDPR), or the restriction of processing (Article 18 GDPR), and the right to object to processing (Article 21 GDPR). Under the conditions set out in Article 20 GDPR, I have the right as a data subject to data portability of the personal data concerning me that I have provided to the Controller.
3.2. As a data subject, I can contact the Controller regarding the processing of my personal data at the email address
3.3. If I, as a data subject, believe that my personal data are being processed in violation of legal regulations, I have the right to request rectification from the Controller. If the request is found to be justified, the Controller shall immediately rectify the defective condition. This does not affect my ability as a data subject to lodge a complaint directly with the Office for Personal Data Protection.
3.4. I hereby give my consent to the processing of my personal data by the Controller for the purposes, to the extent, and for the period specified above. I acknowledge that the Controller will archive this consent for the purpose of fulfilling the legal obligation of the Controller to be able to prove that I have given my consent to the processing carried out.
