CONSENT TO THE COLLECTION AND PROCESSING OF PERSONAL DATA

in accordance with Act No. 101/2000 Coll., on the protection of personal data and Regulation (EU) 2016/679 of the European Parliament and of the Council

By submitting this form and "clicking" on the option I agree, as the subject of personal data, I grant my consent to the processing of my personal data listed below, which I hereby voluntarily provide to the personal data controller, in the manner specified further in this consent. I acknowledge that these personal data concerning me constitute so-called personal data within the meaning of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC ("GDPR"). I hereby declare that I am at least 16 years of age and, if not, I confirm that this consent has been expressed or approved by the person who exercises parental responsibility towards me.

1. Administrator

The administrator of personal data is J. A. CLEAN spol. s r. o., with its registered office at Průmyslová 1888, 258 01 Vlašim, ID: 24736554, registered in the Commercial Register kept by the Municipal Court in Prague C 170064 ("Administrator").

2. Processing of personal data

2.1. My personal data, which I voluntarily provide to the Administrator of personal data, are as follows: Name, Surname, E-mail, Telephone number.

2.2. I consent to the processing of my above-mentioned personal data for the purposes of:

1. marketing purposes of the Administrator related to the offer of products and services; sending information about organized events, products, services and other activities (e.g. by sending newsletters, telemarketing); contacting for the purpose of market research and marketing research; contacting for the purpose of Christmas and Easter or other holidays wishes and sending discount vouchers, gifts, etc.;

whereas the processing of personal data is necessary to fulfill these purposes.

2.3. The legal basis for processing is Article 6(1)(a) of the GDPR.

2.4. The Administrator processes personal data that are adequate, relevant and limited to the necessary extent in relation to the purpose for which they are processed.

2.5. The above personal data will be collected and processed by the following entities in addition to the Administrator:

Recipients of personal data: The Administrator is authorized to transfer the personal data of the data subject in particular to the following recipients and categories of recipients: suppliers of the Administrator, employees of the Administrator, providers of transport services, providers of marketing and advertising services, financial institutions, state authorities within the framework of fulfilling legal obligations set out in relevant legal regulations and other persons in another contractual relationship with the Administrator.

Personal data of data subjects will not be transferred to third countries;

2.6. The Administrator is authorized to process the above personal data for a period of 5 years from the date of my consent. I acknowledge that before the expiration of this period, the Administrator may contact me to renew my consent and that without its renewal, my personal data will be permanently deleted after the expiration of the above period.

3. Rights of the data subject

3.1. I acknowledge that as a data subject, I have the right to request from the Administrator access to my personal data (Article 15 GDPR), their correction or deletion (Article 16 or Article 17 GDPR), or restriction of processing (Article 18 GDPR) and the right to object to the processing carried out (Article 21 GDPR). Under the conditions set out in Article 20 of the GDPR, I, as a data subject, have the right to the portability of personal data concerning me that I have provided to the Controller.

3.3. If, as a data subject, I believe that my personal data are being processed in violation of legal regulations, I have the right to contact the Controller with a request for rectification. If the request is found justified, the Controller shall immediately eliminate the defective condition. This does not affect my possibility, as a data subject, to file a complaint directly with the Office for Personal Data Protection.

3.4. I hereby consent to the processing of my personal data by the Controller for the purposes, to the extent and for the period specified above. I acknowledge that the Administrator will archive this consent for the purposes of fulfilling the Administrator's legal obligation to be able to prove that I have given my consent to the processing being carried out.
